Privacy and contact preferences.

This form collects your name, contact details, care ZIP code, preferred contact method, desired start timing, location and payment path, consent choices, and basic campaign source information.

Why we collect it

Desire uses these details to respond to your request, route it to the correct location and service discussion, and manage follow-up. If you choose optional reminders, you may receive up to two inquiry follow-up emails. You can stop reminders using the link in those emails or by contacting Desire.

Keep sensitive records out of this form

Do not submit medical records, Social Security numbers, Medicaid identification numbers, or financial documents. This is an initial contact form, not a secure clinical intake or a Medicaid eligibility application.

Optional advertising measurement

On our private-duty pages, you can allow or decline Google advertising measurement. If you allow it, Google may receive browser and device information, advertising identifiers, the page visit, and whether a request was completed. We do not send your form fields, care preferences, payment information, or contact details to Google. Enhanced conversions and ad personalization are disabled. Medicaid, GUIDE, and staff pages do not load this advertising tag.

Google measurement is not loaded before you allow it. Change your choice using “Ad measurement choices” in the private-duty page footer. We store that preference in your browser for up to 180 days. Declining does not affect your ability to request care. Read how Google uses information from partner sites.
